Partnerships & Pathways Developed

At ABL we believe that everyone has the right to good health. Our aim is to reduce health inequalities by supporting individuals and communities to lead healthier and happier lives.

Doing Things Differently

From its very inception in 2009, community driven services have been at the heart of everything we do. We are industry leaders in taking services traditionally delivered in a secondary care setting and delivering them in the community. We design services with communities, not for communities.

For over 10 years, we have been taking a whole systems approach to service delivery, working in very close partnership with our system partners.

We are committed to working in a way that gives individuals control over the support they receive.

How We Achieve This

Our person-centred approach generates sustained behaviour change and has been refined over 10 years of improving health outcomes and reducing inequalities across 20+ health and social care systems. ABL’s approach, based on proven methodology (Com-B), supports people to change based on their capabilities, their opportunities, and their motivations.

We are committed to working in a way that puts communities in the driving seat. When it comes to sustainability, we believe citizen-action is more effective than top-down professional intervention. Community-change is built by the people who live, sleep and work there. Extensive training and up-skilling programmes for volunteers, peer supporters and community representatives is always an on-going commitment.

We know having an equitable service does not mean having equal service delivery across a community, which is why we use a proportionate-universalism approach to deliver our services and ensure everybody has the opportunity for A Better Life.

Working together we believe we can provide everyone with the opportunity to achieve A Better Life.

Expertise On Demand

Our coveted ‘My Story’ approach facilitates trust and rapport and helps motivate individuals to address multiple issues in a concurrent or sequential manner at a pace that suits them.

This approach negates disengagement caused by back-and-forth referrals between services and helps build seamless pathways within our own healthcare services and the wider health and social care system.

Our teams have a range of professionals from a number of fields who all work to provide each member, community or partnership the best possible care and attention.

Data At The Heart

At ABL, we strive to gather comprehensive data to fully understand the services we provide. By consistently evaluating our methods and processes, we can identify areas of success and areas for improvement. This ensures that our members receive the highest quality care through expert programmes that are constantly evolving.

We recognise that while data and statistics are important, it is also crucial to understand the overall journey of our members. Therefore, we collect multiple streams of data, including demographic information, socioeconomic status, lifestyle factors, and other factors that can affect our members’ health outcomes.

By considering all these factors, we can tailor our services to meet the specific needs of our members, resulting in more personalised care. This approach leads to better health outcomes, improved member satisfaction, and ultimately, a more efficient delivery system.
